IMPORTANT INFORMATION:  ALL STUDENTS MUST HAVE A PHYSICAL DATED 2/1/16 OR MORE RECENT FOR THE 2016-2017 SCHOOL YEAR. STUDENTS MUST ALSO HAVE ADEQUATE INSURANCE, AND HAVE IT LISTED WITH POLICY # ON THE EMERGENCY MEDICAL CONSENT FORM.

According to MSHSAA By-Laws 3.8.1 and 4.5.6, schools shall require every student participating in spirit activities (sideline cheer and dance teams) and athletics to present a physician’s certificate stating that he or she is physically able to participate.    These By-Laws do not allow for any exceptions to this requirement.
